Creating an assessment

The Create assessment card is the closing step of the assessment flow. Clicking it generates the assessment document — anamnesis, scored questionnaires, and the assessment write-up — and downloads it as a single DOCX file. There is no editable draft stage inside Dive: the DOCX is the assessment, and you take it from there in Word (or the tool of your choice) to refine and share.

Make sure these are in place:

  • The Anamnesis session has been recorded and has content saved.
  • Every questionnaire you want included shows a score (not a “scoring” state).
  1. From the patient’s Assessment tab, click Create assessment.

  2. A spinner appears for a few seconds while Dive builds the document.

  3. The DOCX downloads automatically. The filename includes the patient’s first and last name.

In rare cases an export can come back empty (network blip, scoring not finished). Dive returns an error instead of a blank file. If you hit this, wait a moment and try again; if it persists, contact support.
