Filling out questionnaires
Questionnaires are the structured, scored part of an assessment. Dive currently supports two instruments: AQ and CAT-Q-H. You fill them in inside Dive, hit submit, and Dive scores them in the background — the scores then flow into the Create assessment step.
Open a questionnaire
Section titled “Open a questionnaire”-
From the patient profile, open the Assessment tab.

On the Questionnaires card, click New questionnaire (or open an existing draft).

Pick the instrument (AQ or CAT-Q-H) and click Open.
Filling it out
Section titled “Filling it out”- Answer one item at a time. Progress is shown at the top of the form.
- A draft is saved continuously in your browser. You can close the tab and come back to the same questionnaire — your in-progress answers are still there on the same device.
- Long instruments are fine to split across two sittings; the draft makes that comfortable.
Submitting and scoring
Section titled “Submitting and scoring”When you click Submit, the questionnaire is sent for scoring. Scoring runs asynchronously and usually finishes in a few seconds; the card shows a “Scoring” state until the result is ready. If the result hasn’t arrived after a while, Dive switches to a slower poll in the background — you can leave the page and come back, the score will be there.
Once scored, the questionnaire is locked. To make a change, open a new attempt on the Questionnaires card; the new attempt supersedes the previous one for the purpose of the assessment.
